Duplicate Driving License and Change of Particulars

Obtaining a duplicate of the driving license

  1. Copy of the complaint lodged at the police within 6 months of the loss of driving license.
  2. National Identity Card or the valid passport with the national identity card number
  3. The distorted / defaced driving license, if available.

Change of Particulars

  1. Current driving license.
  2. National Identity Card or the valid passport with the national identity card number
  3. Birth certificate / marriage certificate / Divorce  certificate or any other valid document for changing the name.
  4. Grama Niladhari Certificate for altering the address
